Directions to the Arizona State Board of Dental Examiners



We are located just North of Camelback Road on 19th Avenue.

The 5060 Building is a four story building which sits just behind the 5050 Building
on the west side of 19th Avenue.

 The Board of Dental Examiners is on the
4th Floor with offices in Suite 406 and the conference room in Suite 410.
